Title: Exeptional->elite item

I'm trying to make an Exceptional Sword
into Colossus Blade and the recipe says
1 item + body part + moderator doll + perfect gem
Is this recipe wrong? what should I put in to make this work?

It channged, the ReadMe is a little bit old, you only have to put a Moderator-Doll in the Cube and the item you wish to upgrade. Thats all :)
