Recipe for Puppet box [solved] ?

Hi all,
    I have scoured this site for the latest recipe to make the Puppet Box (Doll Stash) and am unable to find it. The old recipe of Wirt's leg and 2 Horadrin scrolls does not work. Could someone graciously reveal the new recipe please.

Edit. I found a reference that indicates its in the Crypt and indeed thats where i found it after killing a monster.
