Topic: Way out of hell - Baal.

Sorry for spamming, but there is another strange thing. I couldn't of course find proper answer, so I decided to write new post.
I killed at least Baal, and he didn't dropped Way out of hell. He had only You are My hero, but I couldn't take it anyway, because I had one from Damien, who I killed earlier in act 3.
I went to Baal again, and he didn't dropped you are my hero nor Way out of hell.
My question is from where can I get this little piece of bread?
