Horadric Staff frustrations

I've been enjoying this mod tremendously, and recently updated my game from 7.66 sp to the latest version 7.71.   However, i still cannot obtain "with" to complete the horadric staff.  I have read that Dark Elder in the lost city is supposed to drop it, but I've done at least six runs against him and he always drops, "four."  Help!!!
