Are the +all skills mods on a crafted item exclusive of the +class skills mod?

e.g. can we craft scissors katars with +2 all skills and +2 assassin skills? Or a flail with +2 all and +2 barb?

Yes you are right brian.
+2 all +2 Pala or +2 all +2 Barbar flails and +2all +2 Assa katars are possible.

Are there any external attributes to the crafting that increase the chance of +2/+2?

e.g. the level of the character doing the crafting, doing it in Normal vs NM or Hell, etc?

the difficutly should not change anything

with the level i am not sure right now but i think crafting had nothing to do with the characterlevel

afaik there is no way of increasing these chances

clvl and ilvl modify the available affixes on crafted items. As +2 requires the highest ilvl in the game (in D2LOD, at least), however, there is no way to increase your chances :(
